How To: Enroll in your ConnectiCare HSA Solution
Before enrolling in a health plan, it's important to learn about the benefits, features, and costs. To learn more about the HMO HSA-Compatible Plan, review the information provided by your employer.
To enroll in the HMO HSA-Compatible Plan, use the materials and instructions provided by your employer.
Once you are enrolled in the HMO HSA-Compatible Plan, you may be eligible to open a Health Savings Account (HSA). To enroll, use the materials and instructions provided by your employer. Return your completed forms to your Employee Benefits Administrator.
Pre-tax payroll contributions are an easy and convenient way to fund your HSA. The money comes out of each paycheck pre-tax and is remitted by your employer to be deposited into your account. The required payroll deduction form is available from your Employee Benefits Administrator.
Coverage is provided by and services are administered as follows: In Connecticut: Group HMO and POS coverage, and Individual HMO is underwritten by ConnectiCare, Inc.; Individual POS is underwritten by ConnectiCare Insurance Company, Inc. In Massachusetts: Group HMO and POS coverage is underwritten by ConnectiCare of Massachusetts, Inc.
